Social Prescribers work with alot of community services, volunteer groups and the local authority.
They can deal with anything from housing isssues to lonliness and isolation.
They work with individuals with complex needs, multiple long term health issues, complex social needs and mental health needs.
They encourage them to take control of their own well being as well as building a framework with them and pointing them in the right direction of who is out their to help them or who they could help if that makes sense, they also try to encourage them to develop their skills.
The new Care Coordinator from Onside Advocacy, I will be here on a Monday and a Tuesday from 8-4pm.
They can help with the below:
Make and chase appointments for Physio, OT, NBT and Dentist.
Social Care referrals.
Early diagnosis of dementia help and advice.
Alzeimers help and advice.
Worcester carers association.
Continious/persistant callers contact.
Repeat DNA on appointments.
General family concerns and help/advice/support.
The above is just an outline of things they can do and they are willing to help in anyway they can and if it's something they can't sort out then they will source someone who can, if you would like their assistance, please contact the surgery.
